On this occasion, my bad. I didn't notice mine wasn't in my bag.
Having yours on hand might make things easier in general though, I guess?

Yeah, we vary person to person about how proactive we are calling for subs 
when we're on the sideline, and people on court aren't going to call for a 
sub until they're tired. Which isn't going to happen early in the game.

We should really be subbing after no more than a few minutes of play 
(maybe as many as 5 minutes, depending?), in general. Earlier is fine, if 
someone needs it, but other than that it's probably the responsibility of 
whoever is on the bench to make sure things keep ticking over.

I also don't know if there was any reason Alwyn didn't sub himself in 
earlier; with only one sub he usually should. We should remember that 
people shouldn't be spending more time on the bench each half (unless 
they want to) than:
3.3 minutes if we have 6
5.7 minutes if we have 7
7.5 minues if we have 8

Call it 4/6/8 to make it easy.

So if you've been on the bench for approaching that much time, definitely 
call for the next sub, unless you really don't want it. Also feel free to 
call for it sooner and split your bench time up. I should definitely be 
subbing twice each half.
People subbing off should try to keep track of who is coming on, so they 
don't yank those people next. We might want to think about subbing Tommo 
at either timeouts or a minute after, or something, just to make sure he 
gets some rest.
