[CDG5] What does PMDT stand for

I have flipped back and forth on this one. Three things make me think that PMDTs might actually be the entries, not the table:

- NKv2 prints (plural) “Converting PMDTs to Areas”, and the correspondence between these two concepts is nearly 1:1
- The flag constants in the early Trampoline are all called PMDT_*
- The table already has a consistent name in Apple’s headers: the PageMap
