Someone's form arrived in the mail a couple of weeks ago. Return
address of Serpentine? I vaguely remember doing a card, but I don't
recall what was done with it. I vaguely remember it being yours.

I don't recall seeing the money at the time.

Perhaps someone who was at that time responsible has a better
memory, failing that we'll just put a new card in the mail for you.
Your account should be happily functional, I don't think anyone has
even gotten around to account locking anyway.
