[committee] UCC Unpaid Accounts

Hi Conrad

Looks like I was on the list of people that were unpaid and my account
was locked. David just unlocked it for me, here is the relevant coke
log line:
Feb 29 22:40:03 mermaid ucc_srv: success - add -2500 cents to grahame

Not sure if you have my form or not, if not I'll drop by and fill one in.
